Early Signs of Diabetes After 40: A 10-Question Self-Check for Malaysians

Three Malaysian men over 40 at a kopitiam, the age when early signs of diabetes appear

Roughly one in five Malaysian adults has high blood sugar. Close to half of them don't know it yet.

That gap exists because the early signs of diabetes after 40 don't feel like a disease. They feel like getting older, working too hard, or eating too well. So people wait. By the time they test, the number has been high for years.

This self-check won't diagnose anything. Only a blood test does that. But if you answer honestly, it will tell you whether a blood test should be this week or can wait. When you're ready, our guide to diabetes screening in Malaysia explains which test to ask for and where to go.

The 10 questions

Give yourself 1 point for every "yes".

  1. Do you feel heavily sleepy within an hour after lunch, most days? Not mildly full. Struggling to keep your eyes open.
  2. Do you wake up two or more times a night to urinate? And has it got worse over the past year?
  3. Are you thirsty or dry-mouthed even after drinking water?
  4. Do your hands or feet tingle, go numb, or cramp, especially at night?
  5. Has your vision gone blurry on and off, even with glasses?
  6. Do small cuts, scratches or mosquito bites take more than a week to heal?
  7. Has your waist grown while your arms and legs stayed the same or got thinner? Belly fat with thinner limbs is a classic pattern of insulin resistance.
  8. Does anyone in your immediate family (parents, siblings) have diabetes?
  9. Do you drink two or more sweetened drinks a day? Teh tarik, kopi with condensed milk, packet drinks, bubble tea all count.
  10. Is your blood pressure or cholesterol already high? The three usually travel together.

What your score means

0 to 2: Low concern right now. Test once a year after 40 anyway. It's cheap and takes five minutes.

3 to 5: You're showing several early signs of diabetes. Get a fasting blood sugar test within the next month. Don't wait for the next company health screening.

6 or more: Test this week. This many signs together is rarely a coincidence. The good news is that catching it at this stage, before the reading is far into the diabetic range, is where you have the most room to change the outcome.

Why the early signs of diabetes cluster after 40

Three things happen around this age. Muscle mass starts to drop, and muscle is where most of your blood sugar gets used. Daily activity drops, because work and family take over. And the years of teh tarik and late dinners start adding up. Each one on its own is manageable. Together, they wear down how well your cells respond to insulin.

That's the common thread behind every question above. The sleepiness, the night-time peeing, the belly, the numb toes. All of them are what high blood sugar looks like from the outside when insulin sensitivity has dropped.

What to do with a high score

Get the number. In Malaysia, a fasting reading above 7.0 mmol/L is diabetic range. Between 6.1 and 6.9 is prediabetic, the stage where change is easiest.

Cut sweet drinks first. Question 9 is the fastest lever on this list. Kurang manis, then kosong.

Walk after dinner. Fifteen minutes. Muscles clear sugar when they move.

Work on insulin sensitivity, not just the number. This is the part diet alone struggles with.
